Cover only applies if you are not travelling contrary to the advice of your local Government, or for UK residents - the Foreign Commonwealth and Development Office (FCDO). There is a 48 hour waiting period from the date of policy issue before the insurance takes effect as a fraud prevention measure. New Claims requiring inpatient treatment which can be independently verified, can be made within the first 48 hours.
There is no 14 day cooling off period when you are already travelling, so no refund is available in the event you cancel your policy.
